Become A Patient

Hope Health welcomes new patients daily. The first step to becoming a Hope Health patient is to complete the necessary registration forms. Registration forms can be completed and/or printed here. You may also pick up a registration packet in person at one of our facilities. Completed registration forms will be processed within 3-5 business days at which point you will be contacted to schedule an appointment.
